Covid Precautions

We request that Clients do not attend their sessions if exhibiting symptoms of COVID-19 or feeling unwell. We will discuss rescheduling or freezing sessions should a quarantining period be required. All Clients will need to bring a sweat towel and water.

My commitment to you

Confidentiality

All information will be kept in confidence. Your personal details and medical information will not be made available to third parties unless permission to do so is granted by you.

Recommendations

In order for you to maximise your progress, it is recommended that you follow guidelines in relation to healthy eating, sleep, unsupervised training sessions and any other healthy lifestyle behaviours which are recommended to you.

Referral

I intend to work with you within the scope of my knowledge and competences. Therefore, where I believe it is in your best interests to see another health professional, I will make recommendations appropriately.

If I (the trainer) cancels or runs late

Unforeseen circumstances may arise which require our sessions to be rescheduled. In this event the following policy will apply;

  • If I am forced to cancel our session I will reschedule as soon as possible.
  • If I run late in starting our session, I will extend the session time accordingly or make it up at a future date, whichever is most convenient to you.

Your responsibilities to me

Disclosure of information

Please disclose all health information as requested and keep me updated and informed of any changes to your health status. This includes all medical conditions, physical and mental, injuries, allergies and medication you are taking. If necessary, you may need to seek clearance from you doctor before participation in the exercise programme I recommend.

Accepting potential risks

The purpose of an exercise programme is to help you to achieve your health and fitness goals. Your programme will be based upon your present activity/exercise levels and the goals that you wish to achieve. You will experience feelings of exertion during each session and will become hot and uncomfortable at times. Your breathing rate and heart rate will increase as a result of these activities as would be expected from physical exercise.

During your exercise programme, every effort will be made to assure your safety. However, as with any exercise programme there are risks, including cardiorespiratory complications and musculoskeletal injuries. In volunteering for this programme, you agree to resume responsibility for these risks and waive any responsibility for personal damage. You also agree to your knowledge, you have no limiting physical conditions or disability that would preclude an exercise programme.

During your sessions with me you will be closely supervised. During sessions where I am not present you will be responsible for your own safety. If, at any time, you feel undue pain or excessive discomfort stop the activity and inform me of your symptoms.

By signing below, you accept full responsibility for your own health and well-being and you acknowledge an understanding that no responsibility is assumed by your trainer. You confirm that you have read and understood this consent and that you have completed the PAR-Q to the best of your knowledge.

If you (the client) cancels or runs late

  • Personal training sessions that are not rescheduled or cancelled 24 hours in advance will result in forfeiture of the session.
  • If you (the client) are late to a session, the remainder of the scheduled session time will be delivered, unless other arrangements have been previously made.
  • No personal training refunds will be issued for any reason, including but not limited to relocation, illness and unused sessions.

Communication and Commitment

While I can give you the tools and encouragement to reach your goals, ultimately you will need to assume responsibility for making the required changes. I can best help you with this process if you keep me fully informed of any issues that arise for you and if you have any questions or concerns please do not hesitate to discuss them with me.
